A properly indexed file generally contains:
- Covering letter
- Document index
- DMB certificate
- Court order
- Insurance policy
- Surrogate mother documents
- Medical fitness certificates
- Consent forms
- Affidavits/undertakings
- ART & Surrogacy clinic registration certificates
- Application forms for Appropriate Authority
In Punjab, many Appropriate Authorities prefer three separately indexed files:
- File 1: Intending Couple
- File 2: Surrogate Mother
- File 3: Clinic & Legal Documents
