Clinics generally arrange the documents in the following sequence before submission to the Appropriate Authority and District Medical Board.
A. Intending Couple File
1. Identity & Address Proof
Under the current Indian surrogacy framework, both NRIs (Non-Resident Indians) and OCI cardholders (Overseas Citizens of India) are generally eligible to pursue altruistic surrogacy in India, subject to statutory conditions. Commercial surrogacy remains prohibited.

2. Marital Documents
- Marriage Certificate
- Joint photograph
- Marriage duration proof (if required)
3. Age Proof
- Birth Certificate / Matriculation Certificate / Passport
4. Eligibility Documents
- Affidavit regarding no surviving child (if applicable)
- Declaration under Surrogacy Act
- Undertaking for altruistic surrogacy
5. Medical Documents
- Complete infertility records
- Previous IVF treatment records
- Relevant specialist opinions
- Medical indication certificate supporting surrogacy application
6. District Medical Board (DMB)
- Application to DMB
- Medical records submitted to DMB
- DMB Certificate recommending gestational surrogacy
7. Court Documents
- Petition for parentage and custody order
- Court order / Magistrate order as prescribed
8. Insurance
- Surrogate mother's insurance policy (36 months coverage)
9. Appropriate Authority Forms
- Form 2 and prescribed declarations
- Application for Certificate of Essentiality
- Application for Eligibility Certificate
